Registration, Post-Test, and Program Evaluation

This course  (ACPE #038-999-00-052-H01) qualifies for 1.0 contact hours (0.10 CEUs) of continuing pharmacy credit, which will be awarded within 4 weeks after submission of a successfully completed program.  A passing grade of 70% is required.  Any participant who fails the examination may be re-examined one additional time. 

In order to apply for continuing pharmacy education credit, please complete the post-test and evaluation, answer the questions directly on the post-test form, and submit via e-mail using the “submit” button at the bottom of the post-test.  The form can also be printed out, placed in an envelope, and mailed to:
